ADMISSION
AND REGISTRATION PROCEDURES

TO DO BEFORE LEAVING HOME |
Before leaving your home country, ERASMUS students must take care of the following:
- Reach an agreement with your faculty about the courses you will take in Milan;
- Procure a certificate from your University testifying to your SOCRATES/ERASMUS status;
- Procure an E128 emergency medical treatment form for medical assistance;
- Procure an Accident Insurance certificate (valid abroad);
- Make sure that your Passport or Identity Card is valid;
- Procure 5 passport-size photographs.
N.B.: Make sure that your University has informed by August 12, 2005, the SOCRATES/ERASMUS Office about your arrival at the Università degli Studi di Milano.
In case this nomination arrive later, you will be accepted as ERASMUS student at Università degli Studi di Milano, but you are not entitled to request accommodation services.

WHEN YOU ARRIVE IN MILAN |
If you are an ERASMUS student, you
should enrol within 2 days of arriving: to do this, hand the Application Form A
(rtf version) into to the E.U. Training Programmes Office - Socrates/Erasmus Section - Via S. Antonio 12 - 20122 Milano (open Monday to Friday 9,00 - 12,00), along with the following documents:
- passport or any other identification;
- 2 passport-size photographs;
- your University's certificate testifying to your SOCRATES/ERASMUS Student status;
- Accident Insurance certificate (valid abroad).
The office will issue a double copy declaration certifying your enrolment: use one copy to apply for the university's library card and the other one to buy a public transport card if you are aged under 26. If you are eligible, take this declaration to the offices of the A.T.M. (Milano Transport Company) together with your identity card or passport and 1photograph. The A.T.M. has offices in piazza Duomo and piazza Cadorna. A monthly season ticket costs about € 17.
The SOCRATES/ERASMUS Office will also give you a personal booklet where a record will be kept of your studies. If no information is available at your own university about the courses you intend to take in Milan before you leave home, you should contact your SOCRATES Departmental Co-ordinator at the Università degli Studi di Milano before coming to Milan.
MEDICAL
ASSISTANCE
ERASMUS students must bring an E128 emergency medical treatment form with them in order to be entitled to the services provided by the Italian Health Service. The E128 form must be requested from your home country's health service, before you leave home. When you arrive in Milan, you should hand it into an office of the A.S.L. (Azienda Sanitaria Locale - Local Health Authority).
There are several A.S.L. offices all over Milan. You should ask the A.S.L. office nearest to where you live for a "Carnet della Salute". This document will entitle you to free medical assistance, i.e. medical day hospital, hospital treatment and prescriptions.
ACCIDENT
INSURANCE (COMPULSORY)
ERASMUS students are recommended to be supplied with insurance coverage against accidents during their whole stay in Milan. The policy should include a coverage applicable to any accident that might occur during didactic activities.
This coverage may be provided by your own University or taken out with a private insurance company.
In either case, you should produce evidence of this insurance when enrolling with the SOCRATES/ERASMUS Office, otherwise you will have to procure one through the University of Milan, paying about € 8,00 into the University's post office account.
RESIDENCE PERMIT
ERASMUS students are recommended to take the necessary steps in order to
obtain the "Carta di soggiorno" through the "Questura di Milano".

ENROLMENT
TO EXAMS
Students may enrol to exams by bringing their
personal booklet, provided by the SOCRATES/ERASMUS Office, to the SIFA
Services of their own Faculties within the deadlines set by the academic
staff.

Please note that these Offices are in charge of
enrolment procedures only.
Information on exams timetables and enrolment is available at the University's
Departments/Institutes.
Before you leave the Università degli
Studi di Milano, you should return your personal booklet to the SOCRATES/ERASMUS
Office. The Office will then issue an official certificate listing the
activities (examinations, laboratory activities, etc.) registered in the
booklet.
If the Office is unable to give you the certificate personally, it will be
sent to your home address or to your University.
Before leaving Milan, you must also complete any tasks set by your Italian
academic tutors.
